Preserve records without exposing sensitive health information
The public form records only broad categories. Hall Attorneys can request source documents through an appropriate secure process if needed.
Notice and relationship records
Keep original notices, account emails, invoices, support messages, employment records, and correspondence from McKesson or a connected pharmacy, provider, insurer, employer, or patient-support service. Preserve message headers when available.
Service and transaction records
Keep redacted billing, reimbursement, delivery, appointment, or service records that establish the relationship. Do not send full member numbers, account numbers, prescription details, diagnoses, or medical records through this public form.
Suspicious activity and cost records
Save phishing messages, login alerts, fraud reports, replacement-card records, expense receipts, service-disruption notices, and a dated log of time spent responding. Keep originals rather than edited screenshots when possible.
Data minimization
Do not provide passwords, one-time codes, Social Security numbers, government IDs, full financial-account numbers, medical records, or detailed health information. A category selection is enough for initial screening.
